Metabolic
Semaglutide
FDA-approved (type 2 diabetes; chronic weight management)
In simple words: A prescription GLP-1 medicine (weekly injection or daily tablet) that lowers appetite and blood sugar.
How it works
It copies the gut hormone GLP-1: boosting insulin when sugar is high, slowing stomach emptying and turning down hunger signals in the brain.
Targets: GLP-1 receptor · Class: GLP-1 receptor agonist
Uses, latest studies & pairings
Typically used for
- Type 2 diabetes (Ozempic injection, Rybelsus tablet), including kidney protection in diabetic kidney disease.
- Weight management and lowering heart attack and stroke risk (Wegovy), plus a form of fatty liver disease with scarring (MASH).
- A once-a-day Wegovy pill was approved for weight management in December 2025.
Commonly paired with
- Strength training and enough protein (about 1.2–1.6 g per kg of body weight a day, unless your doctor says otherwise) to protect muscle while losing weight.
- Not combined with other GLP-1 medicines: they work on the same receptors, so stacking adds side effects, not safety.
- Cagrilintide (an amylin medicine) in the investigational CagriSema combination.
- Fiber, water and electrolytes to ease stomach side effects.
Latest studies & news
- Dec 2025
Oral Wegovy (25 mg pill) approved in the US for weight management, based on trials averaging about 14% weight loss.
- Nov 2025
EVOKE trials: semaglutide did not slow early Alzheimer's disease.
- Aug 2025
Wegovy received accelerated approval for MASH with moderate-to-advanced liver scarring, after the ESSENCE trial: MASH resolved in about 63% of people versus 34% on placebo.
- 2025
Ozempic approved to lower kidney and heart risk in type 2 diabetes with chronic kidney disease (FLOW trial).
What the research shows
Human clinical evidence
Human- STEP 1 (68 weeks, adults with obesity): average weight loss of about 15% with 2.4 mg weekly versus about 2% with placebo (Wilding 2021).
- Large trials also showed fewer heart attacks, strokes and cardiovascular deaths in people with heart disease and overweight (SELECT trial).
- Approved as Ozempic and Rybelsus (type 2 diabetes) and Wegovy (weight management, cardiovascular risk reduction and a form of fatty liver disease, MASH).
Animal and lab results suggest what might happen; they don't prove an effect in people.
Safety & what to watch
- Common: nausea, vomiting, diarrhea, constipation, mostly during dose increases.
- Boxed warning for thyroid C-cell tumors in rodents; avoid with a personal or family history of MTC or MEN 2.
- Warnings include pancreatitis, gallbladder disease, worsening diabetic retinopathy with rapid sugar improvement, and a very rare optic-nerve condition (NAION).
