Survodutide

Also known as BI 456906

InvestigationalHuman clinical evidence

Investigational; in Phase 3 development

In simple words: An experimental weekly injection that acts on both the GLP-1 and glucagon receptors.

How it works

GLP-1 lowers appetite; glucagon raises energy use and pushes the liver to burn stored fat, which is why it is being studied for fatty liver disease.

Targets: Glucagon receptor, GLP-1 receptor · Class: Dual glucagon / GLP-1 receptor agonist

Uses, latest studies & pairings

Typically used for

  • Obesity and overweight.
  • Fatty liver disease (MASLD and MASH with scarring), where its glucagon action on the liver may give it an edge.

Commonly paired with

  • Strength training and enough protein (about 1.2–1.6 g per kg of body weight a day, unless your doctor says otherwise) to protect muscle while losing weight.
  • Not combined with other GLP-1 medicines: they work on the same receptors, so stacking adds side effects, not safety.

Latest studies & news

  1. Jun 2026

SYNCHRONIZE-1 (725 adults, 76 weeks): up to 16.6% average weight loss versus 3.2% on placebo. In a substudy, visceral (deep belly) fat fell by up to 34% and liver fat by up to 63%. Side effects were mostly stomach-related.

  2. Jun 2026

SYNCHRONIZE-MASLD (218 people, 48 weeks) also met its main goals, published in Nature Medicine.

  3. 2024

Phase 2 MASH trial: up to 83% of people had their MASH improve, versus 18% on placebo.

  4. Next

LIVERAGE trials in MASH with scarring are due to report. Not yet filed for approval.

What the research shows

Human clinical evidence

Human
  • Phase 2 trials reported meaningful weight loss and improvements in liver fat and MASH.
  • Phase 3 SYNCHRONIZE-1 (2026, 76 weeks): up to 16.6% average weight loss versus 3.2% on placebo, with large cuts in liver and visceral fat.

Animal and lab results suggest what might happen; they don't prove an effect in people.

Safety & what to watch

  • Gastrointestinal side effects were the most common in trials.
  • Not approved; investigational only.

Amounts

No dose is given for unapproved substances. If you are considering it, discuss it with a licensed clinician who knows your history and bloodwork.
